Article 299. Contents of the application

The following shall be specified in the application for adoption:

Surname, name, patronymic of the adoptive parent/s, place of their (his/her) residence;

Surname, name, patronymic and date of birth of the adopted child, his/her place of residence (location), information about the parents of the adopted child, the presence of brothers and sisters;

Circumstances justifying the request of the adoptive parent/s for adoption and the evidence confirming these circumstances;

Request to change the surname, name, patronymic of the adopted child, date of birth (not to exceed one year), place of birth of the adopted child (if the child is not more than ten years old); to record the adoptive parent/s in the birth record of the child as the parent/s should the adoptive parent/s wish to make appropriate changes in the birth record of the child.

The application for adoption shall be signed by the adopters (adopter).
